Nomination is open to both academic institutions and public. Each nomination must be sponsored by two nominators. It should include a brief resume of the nominee, outlining his or her achievements and/or contribution. All nominations should reach Roslyn M. Tsao, President of the FCCP Education Foundation at 1108 - 45 Carlton Street, Toronto, ON M5B 2H9 by July 31 of the year.
